For Professionals
Salubre Care partners with physicians, therapists, treatment programs, and other professionals who need specialized help caring for patients with substance use disorders, medication dependence, and complex behavioral health needs.
Sometimes the need for a referral is clear. Other times, the situation is less straightforward: a chronic pain patient is having increasing difficulty managing prescribed opioids; a surgical patient cannot taper pain medication as expected; a psychiatric patient has developed dependence on benzodiazepines; alcohol or substance use is interfering with treatment of another medical condition; or a primary care physician is concerned about a patient's substance use but is unsure whether it meets criteria for a substance use disorder.
In these situations, Salubre can serve as both a consultant and a treatment partner.
When to Consider Salubre
Healthcare professionals may consider consultation or referral when a patient:
- Drinking more or longer than intended
- Needs medically supervised withdrawal or evaluation for home detoxification
- Has difficulty reducing or stopping alcohol, opioids, benzodiazepines, kratom, or other substances
- Develops concerning patterns around prescribed controlled medications
- Has chronic pain complicated by opioid dependence or possible opioid use disorder
- Has persistent opioid use or difficulty tapering following surgery
- Has substance use that is complicating psychiatric or medical treatment
- Has co-occurring addiction, psychiatric symptoms, chronic pain, or a complicated medication regimen
- Is transitioning from detoxification or residential treatment and needs ongoing physician-led care
- Needs an addiction medicine evaluation to clarify diagnosis, risk, and appropriate next steps
Referral to Salubre does not necessarily mean transferring a patient's care.
In many situations, the referring physician or therapist remains an important part of the patient's treatment team. With appropriate patient permission, we can evaluate the addiction or medication-related concern, provide diagnostic and treatment recommendations, manage a specific component of care when needed, and communicate with the referring professional so that existing therapeutic relationships can continue.
Depending on the patient's needs and clinical appropriateness, Salubre services may include addiction medicine consultation, medically supervised home detoxification, medication-assisted treatment, psychiatric evaluation and medication management, medication tapering and deprescribing, recovery monitoring, telehealth, and concierge behavioral health care.
We also welcome appropriate inquiries from behavioral health professionals, treatment programs, attorneys, EAPs, and other professionals seeking clinical evaluation or treatment resources for individuals they serve.
Not Sure Whether a Referral Is Appropriate?
You don't need to determine the diagnosis before contacting us. If substance use, medication dependence, withdrawal, or behavioral health concerns are making a patient's care more difficult, our team can help determine whether Salubre is an appropriate resource and what level of care may be needed.
