Complex Care Coordination

Bringing the Right People Together Around a Complex Problem

Some health and recovery challenges cannot be solved by a single prescription, appointment, or provider.

Addiction may exist alongside anxiety, depression, chronic pain, insomnia, relationship difficulties, physical deconditioning, or other medical and behavioral health concerns. A difficult medication taper may require more than medication management alone. And families trying to help someone they love can find themselves navigating multiple providers, treatment recommendations, and levels of care without anyone looking at the whole picture.

Salubre Care provides complex care coordination and case management for selected patients with complicated addiction, medication, and behavioral health needs, helping patients and families identify barriers to progress, determine who should be involved, and bring the different pieces of care together.

A Recovery Quarterback

In complex situations, Salubre Care can serve as a Recovery Quarterback—helping build the team, coordinate the larger plan, and keep attention focused on the needs and goals of the patient.

The appropriate team is different for every person. Depending on the situation, it may include:

  • Addiction medicine, primary care, psychiatric, or specialty physicians
  • Therapists and counselors
  • Physical therapists
  • Dietitians and nutrition professionals
  • Personal trainers and other wellness professionals
  • Recovery professionals and peer support
  • Spouses, family members, and other trusted supports
  • Pastors or spiritual advisors when faith is meaningful to the patient

Some of these professionals may be part of Salubre Care. Others may be independent providers already known to the patient or specialists we help identify.

The goal is not to keep every aspect of care within Salubre. The goal is to build the right team around the patient and the problem.

Helping the Pieces Work Together

When appropriate and with the patient's permission, our role may include communicating with members of the care team, helping patients and families understand recommendations, identifying gaps or barriers to progress, and coordinating the next steps in care.

This can be particularly valuable when a patient is transitioning from detoxification or a period of crisis into longer-term recovery, when several medical or behavioral health concerns are occurring at once, or when previous attempts at treatment have addressed individual symptoms without addressing the larger picture.

Our involvement can change as the patient's needs change. At times, Salubre may take an active role in coordinating several aspects of care. At others, we may simply help identify the next right professional or resource and make the connection.

One patient. One larger picture. The right people working toward the same goal.
